About...
The Community Outreach Assistance Team (C.O.A.T.) provides services to improve the health of children and families especially among racial and ethnic minority groups. The program works to assure pregnant women are able to receive prenatal care so their babies are born healthy and that children are able to receive pediatric care so that they stay healthy.
Program Services
C.O.A.T. Care Coordinators work hard to find pregnant women and women with babies that need help with their health and social service needs. They work closely with the women they serve to link them to needed health and social services, offer support and assistance for health care appointments, provide education on health issues and help them with any barriers that keep them from getting needed health care.
Who Is The Program For?
C.O.A.T. Care Coordinators work with pregnant women and women with children less than 1 year of age.
How To Use The Program
Women may call (614) 645-7068 for an appointment in their home or another location.

Community Care Coordinators work with women in their own homes or other locations as arranged.
Hours
Weekdays 8:00 am to 5:00 pm and by appointment.
