Preparedness, asking the right questions, effective communication with providers, and active participation in decision-making plays an important role in achieving health and well-being. Improving your health starts with you. This workshop gives you the tools you need to partner with your doctor by preparing for your visit, what to do while at your visit, and how to follow-up after your visit.
Program Services
This 2-hour workshop provides a workbook of tools and local resources. In the workshop, you will understand the importance of and how to: PREPARE - Know your health history
- Know your family health history
- Research your health condition
ASK - Take a list of questions to ask the doctor
- Ask questions during your doctor visit about anything you do not understand
TELL - Your health history (operations, medication, problems)
- What has occurred between visits
- How you are feeling now
FOLLOW-UP - Keep your appointments
- Arrive early for your appointment (Be on time)
- Follow the treatment plan
- Ask for clarification of anything you do not understand
- If you are having problems, call your doctor

The workshops are free.
Location
Workshops are offered at various places around Columbus. They have been held at libraries, senior housing complexes, churches, and recreation centers. Call to find the workshop closest to you or to arrange one for a group.
