About...

Child\'s faceA Fatality Review is a community planning process in which a team of experts from various community systems and agencies convene to review the circumstances around potentially preventable deaths (e.g. all children).  This review process provides a tool to identify common themes and trends among cases, identify gaps in community systems, and develop recommendations for system improvement to reduce preventable deaths.

Program Services

The Columbus Public Health's Child Fatality Review Program convenes and supports the state-required Franklin County Child Fatality Review (CFR) Team. The CFR Team is comprised of representatives from the coroner's office, law enforcement, child protective services, public and mental health, a pediatric physician, and other community organizations.

sleeping childTo support this process, Columbus Public Health (CPH) program staff:

  • Identify and prepare cases for review
  • Complete background research about cases
  • Organize and facilitate monthly Child Fatality Review Meetings and case reviews
  • Report county-level data to the State each year
  • Analyze data and trends to identify opportunities for preventing future child deaths
  • Assist the FC CFR Team in developing recommendations for system improvement and community prevention strategies

Fatality review staff work with other CPH programs and community organizations to disseminate fatality review recommendations, monitor implementation, and identify ways to reduce preventable deaths in our community.
