The Help Me Grow program assures that newborns, infants, and toddlers have the best possible start in life. Every family that is eligible and provides consent for ongoing Help Me Grow services receives services is assigned to a Help Me Grow Service Coordinator who works with the family through home visiting. The Service Coordinator works with each family through an Individualized Family Service Plan identifying family goals, linkage to services, mentoring, family support and advocacy.
Program Services
Help Me Grow Service Coordinators are licensed social workers who link with families to assure that children age birth to three who are at risk for, or identified with developmental delays or disabilities have access to and are linked with needed and appropriate services.
Who Is The Program For?
Pregnant women and families with children age birth to three who are at risk for developmental delays or disabilities or Part C eligible children (children identified eligible due to mental retardation and/or developmental delays.
How To Use The Program
Pregnant women, parents, health providers and community agencies can refer to Help Me Grow of Franklin County referral center by calling (614) 227-9860. The family is then referred to an agency in Franklin County for Service Coordination services. CHD is a Help Me Grow of Franklin County Service Coordination provider.
Cost
No charge for eligible families.
Location
Help Me Grow Service Coordination is provided in the home by Service Coordinators assigned to the family by the Help Me Grow referral staff.
Hours
Based on scheduling between Service Coordinator and referred family.
