| Healthier, Safer People Honor Award |

For the first time, Columbus Public Health will be recognizing our outstanding licensees with the "Healthier, Safer People Honor Award". The first annual Honor Awards will be given out this spring. For your licensed food facility to qualify for the Honor Award, your facility: 
| - Must have had a valid food facility license from 1/1/2007 through 12/31/2007.
- Must have properly applied for the 2007-2008 license on or before the required due date.
- Must not have required any follow-up inspections in 2007.
- Must have had at least one Person in Charge attend food safety training in 2007.
- Must not have had any Smokefree violations.
- Must be in good standing with other city agencies.

If you think your licensed food facility is qualified for the 2007 Honor Award, and you haven’t already submitted the pink education form that was mailed with your license application, please complete the form linked to below and get it back to us before March 14, 2008.
